Community Therapy Team

The community domiciliary service for adults covers a wide variety of conditions. Referrals for assessment and treatment are accepted from health and social care professionals across Leicester, Leicestershire and Rutland., as well as self referrals from patients directly. There is joint working with Occupational and Physiotherapists, District Nurses and unregistered support staff within the locality hubs alongside Social Workers and other members of the Multidisciplinary Team externally.
Team consists of 2 elements, Planned Therapy and Home First.
Home First brings together an integrated health and social care offer delivered by community-based Nursing, Therapy and Adult Social care teams.
Home First:
• supports people to remain in their homes when they are having a health or social care crisis, rather than needing to go into hospital or a care home.
• helps people get home from hospital quickly and provides them with rehabilitation and reablement to help restore their health, wellbeing and independence.
• Discharge 2 Assess – supporting patients with ongoing rehabilitation when interim discharge to a Residential Home has been part of their discharge pathway prior to returning home.
• We also have specialised Bariatric and High Dependency Discharge to assess offers to support patients with additional needs have the right time and environment to assess their function to hopefully return home
The service can be contacted 24 hours 7 days a week with health and social care staff offering an integrated response. The service will deliver a home based crisis response within 2 hours (where clinically required) and can start home reablement within 2 days. The length of time patients will be cared for by the service will vary from only one or two visits being required up to six weeks.
Planned Therapy are routine referrals for non-urgent ongoing rehabilitation needs, not at immediate risk of deterioration. This service aims to deliver Therapy within 18 weeks of initial referral.
Conditions Likely to See:
Respiratory (non acute), existing neurology patient such as Parkinsons and Strokes, orthopaedics and palliative, patients in crisis with an emphasis on the elderly.
Each hub has parking on site or nearby
Each hub will have hot and cold drinks with an onsite kitchen and staff area or local shops, some of the hospital-based hubs have an onsite café but please note this varies and we recommend bringing your own lunch on the first day.
Inductions will be supported locally by the placement manager at the site the placement is at
